Welcome to your first visit! Please arrive 15 minutes prior to the initial evaluation. You may be asked to complete a few more forms. Be sure to bring the following items with you: Insurance Card, IEP (if applicable), IFSP (if applicable), Hearing & Vision Screening Results, Previous evaluations from other medical professionals.
